Meu nome é or eu me chamo?
Both, and neither is more correct. Meu nome é is the plainer one and the easier to say. Eu me chamo is the reflexive version, literally I call myself, and matches the question como você se chama?
Onde fica or onde está?
Onde fica for things that do not move: a street, a bathroom, a station. Onde está for something that could be somewhere else right now. This set uses fica, because you are asking about places.
